      <abstract xml:lang="en">
        <p>Population ageing presents new challenges for the care and wellbeing of older adults. The “CuraMI &amp; ProteggiMI” Project, initiated by the Ravasi Garzanti Foundation, aims to establish an integrated network of services that ensures person-centred assistance, while also undertaking a comprehensive evaluation of the individual’s relational system, supports – in essence, the ‘ecosystem’ to which the person belongs. At the heart of the initiative lies the individual, around whom increasingly wider circles are drawn, ultimately creating a clear map that contextualises requests for support and enables the reconfiguration of life trajectories, as well as the preservation of bonds and relationships, even in the face of profound vulnerability. By fostering collaboration among various actors within the health and social care sectors and by embracing a holistic approach, the project seeks to enhance the quality of life for both older persons and their caregivers. The adopted methodology involves rethinking home care services, co-designing with stakeholders, and promoting shared service provision. The initiative is implemented through physical help desks and digital tools designed to offer informational support, guidance, and personalised assistance. The success of the project relies on the creation of strategic alliances, institutional recognition, and a continuous commitment to research and innovation, in order to respond effectively to the evolving demands of longevity.</p>
